Every doctor had a form for me to fill out questioning the chronology of tests, who performed the tests and the results. It always seemed no matter how much information I carried with me, there was always some other record I hadn't realized was needed. Not only was I struggling with cancer, I was struggling with the paperwork and scheduling of appointments necessary to fight the disease. It was endless and daunting.
